ODS Individual Options Dental Plan

Looking for a dental plan to add to your ODS individual health benefit plan coverage? If so, this is the plan for you. This dental plan gives you the freedom to choose your own dental professional, and there is no waiting period or deductible for preventive services such as exams, x-rays and cleanings.

Eligibility Requirements**

1. To be eligible for this dental plan, you must enroll in any ODS individual health benefit plan. Enrollment in an ODS individual health benefit plan must match enrollment in this dental plan.
2. New ODS individual health benefit plan members are eligible to select the dental plan during a "one time only" enrollment period. This is the only opportunity to select this coverage.
3. The dental plan's effective date will coincide with the effective date of your ODS individual health benefit plan.
4. Dental plan members may not retain this plan if eligibility in an ODS individual health benefit plan terminates. If a dental plan members drops the dental coverage, it cannot be reinstated.

**This dental plan is not available to members who are receiving medical premium subsidy through the Family Health Assistance Program (FHIAP).
