Eligible members must reside in the state of Oregon, and/or reside in specific service areas of the plan.

Individual Plans for Oregon Residents

Moda Health offers a variety of plans for individuals and families residing in Oregon who want choice, quality, and value in their health insurance program. If you are already enrolled, check your ID card to see what type of plan you have. If you are interested in enrolling, Moda Health offers the following types of individual medical and dental plans:

Apex Plan [More Info]
The Apex plan is best for those looking for a higher level of benefits and a lower total out-of-pocket cost. The Apex plan includes services that can be accessed before the deductible, including preventive care, pharmacy services, unlimited doctor’s office or urgent care center visits, and alternative care.

Maximizer Plan [More Info]
The Maximizer plan is ideal for individuals who want broad coverage for a range of services, including pharmacy benefits. The Maximizer includes a $20 co-payment for doctor office visits, alternative care and urgent care centers.

Foundation Plan [More Info]
Our Foundation plan is a great, low-cost option for individuals primarily needing catastrophic coverage in case of unforeseen and serious medical needs, while still providing preventive care benefits with the deductible waived. 

HSA Value Plan [More Info]
The Moda Health HSA Value Plan is their lower priced HSA eligible plan. Premiums are very compelling and this plan features wellness benefits that are not subject to the deductible...

Beneficial Value Plan [More Info]
The Beneficial Value plan is suited to individuals shopping for a lower premium cost. The Beneficial Value plan offers catastrophic coverage and also waives the deductible for preventive care and the first three office and alternative care visits per plan year.

WellConnect Plan [More Info]
Our WellConnect plan encourages members to engage in their own healthcare. Members who seek care within our Community Care Network (CCN) enjoy a higher benefit level and receive high-quality, individualized care from an interconnected group of providers. This plan also offers tiered access to the Moda Health Plus Network.

Individual Dental Plan [More Info]
Looking for a dental plan to add to your Moda Health individual health benefit plan coverage? Our dental plan gives you the freedom to choose your own dental professional with no waiting period or deductible for preventive services.

Underwriting
Applicants must be approved by Moda Health underwriting. Effective dates are assigned by underwriting and are generally the first day of the month following underwriting approval.

Billing
Members have the option to pay monthly by electronic checking account deduction, monthly via billing statement subject to a $2.00 fee per month or quarterly via a billing statement. Premium must be collected and sent in with the application.
